About USMLE 2026

USMLE Service Transition has simplified the journey of examinees in January 2026. All USMLE Step exam services, including exam registration, score report delivery, and customer service, under FSMB and NBME. International students wishing to apply for a residency programme or license in USA must complete the two steps: Step 1, Step 2 CK at usmle.fsmb.org and the final 3rd step at NBME.

What is USMLE? 

All international students wishing to practise medicine in USA must apply and appear for USMLE. It is a medical licensing exam that tests the student's ability to apply knowledge, concepts, and principles as well as demonstrate fundamental patient-centered skills that form the basis for safe and effective patient care.  USMLE is generally taken during the different stages of the medical education.  USMLE is a computer-based test that uses the Computer-based Case Simulation software (CCS). 

USMLE Full Form

The full form of USMLE is the United States Medical Licensing Examination. Held by Federation of State Medical Boards (FSMB) for international students, USMLE is a three-step examination. The exam is under the aegis of Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners(NBME). While USMLE 2026 Step 1, Step 2 CK can be attempted from any major city across the world in USMLE test centres, while USMLE Step 3 must be taken in the US. 

How to practise medicine in USA?

All international medical students wishing to pursue medicine in USA must appear for USMLE. The three step exam acts as a qualifying exam for obtaining the license to practice medicine in the USA. Candidates who are studying MBBS or graduated from medical programs are eligible to take this USMLE 2026 exam. USMLE 2026 will have three steps: Step 1, Step 2 CK, and Step 3.

It is to be noted that all international medical students must determine their eligibility for ECFMG Certification before they can register for the USMLE 2026 exams. The ECFMG certification must be obtained before attempting USMLE Step 3. To qualify USMLe, the sequence is to apply for the ECFMG certification, register for Step 1, followed by Step 2 CK, obtain ECFMG certification and finally appear for the USMLE 2026 Step 3 exam.

USMLE 2026 - Steps and Process

As stated there are 3 steps in USMLE exam, each step assesses the candidate's understanding and application of concepts in the practice of medicine.

  • Create a MyIntealth account and apply for ECFMG Certification
  • Register for USMLE Step 1 exam in FSMB's USMLE Portal (This account is for all USMLE services)
  • Register for Step 2 CK exam in FSMB's USMLE Portal
    Important: Step 1 and Step 2 CK are required for ECFMG Certification
  • Obtain ECFMG Certification
    Note: A valid, unexpired ECFMG Certification is mandatory to enter an accredited U.S. medical training program and take USMLE Step 3.
  • Appy for U.S. residency positions and join
  • Register for Step 3 exam in FSMB's USMLE Portal
  • Finish the residency  and apply for a state medical license

USMLE 2026 Latest Updates

Update 1: In January 2026, the co-sponsors of the USMLE exam namely FSMB and NBME, have centralized all USMLE service functions. This translates to consolidation of all USMLE examinee services: exam registration, score report delivery, and USMLE customer service with FSMB and NBME.  

Update 2: USMLE Step 1 score reporting has transitioned to pass/fail only. For multi-day exams, the date of the first day of the USMLE testing will determine score reporting. Both Scores and Transcripts will include only Pass/Fail outcome.

Update 3: The number of USMLE exam attempts allowed for each Step is now 4 including incomplete attempts. 

Update 4: The USMLE program has released the examinee performance data for each of its three Step exams. The performance tables show the passing rates for each Step by various examinee groups and are available at www.usmle.org/performance-data.

You should possess a DO degree from an AOA accredited medical school which is located in the USA OR an MD degree from WDOMS listed medical schools located outside of the USA and Canada and possess the ECFMG Certification. Q: Is MBBS required for USMLE?

Q: Can I work as a doctor in the USA without USMLE?
A:

To receive your MD degree from a US medical school, you must pass parts 1 and 2 of the United States Medical Licensing Examination (USMLE). The majority of DOs educated in the US take it as well. Additionally, you cannot obtain a medical license without completing USMLE parts 1, 2, and 3.

Q: Can I get residency in the USA without USMLE?
A:

You cannot apply for residency in the US as a foreign medical graduate without passing the USMLE (Steps 1 and 2). You won't be able to apply for a medical license in the US if you don't pass all USMLE components, including Step 3.

Q: How many times can you attempt USMLE?
A:

There can be a maximum of four attempts for each Step (4). Examinees who have taken four or more attempts at any USMLE Step (including Step 2 CS) and failed are ineligible to apply for USMLE Steps.

Q: How many years are USMLE scores valid?
A:

The USMLE scores are valid for 7 years. In the said period, you have to pass the additional ECFMG certification.

Q: Is there an age limit for USMLE?

There is no age limit for USMLE.
